Gujarat Launches PM Family Care Tracker Pilot to Build a Unified Dashboard for Maternal & Child Health

The dashboard enables real-time monitoring of maternal care, newborn health, immunisation, nutrition, growth tracking, school enrolment, attendance and adolescent health services.
The Government of Gujarat has launched the pilot phase of the PM Family Care Tracker (FCT) in Gandhinagar, introducing an integrated dashboard that brings together maternal and child health, nutrition and education records.
Unveiled by Union Home Minister Amit Shah, the platform has been designed to digitally monitor every stage of a child's development, from pregnancy through 18 years of age, while helping eligible families receive welfare benefits on time.
The pilot integrates data from the Health Department, the Women and Child Development Department and the Education Department.
Health records are linked through the Poshan Tracker, while education data is sourced from the Child Tracking System (CTS).
Every mother and child receives a unique digital identity using the ABHA number and Birth Registration Number, enabling continuity of records across different stages of life.
"This dashboard, by integrating various types of data, will deliver the benefits of central and state welfare schemes directly to the doorsteps of needy families," Amit Shah stated.
Gujarat Health Minister Praful Pansheriya said the platform was developed by bringing together the three departments under Shah's guidance.
"Under his guidance, this project is being launched today. It is through his vision that the Gujarat Health Department, the Women and Child Development Department, and the Education Department have been brought together so that data from all three departments can be integrated, and we are launching the PM Family Care Tracker Pilot Project from here," he said.
According to the state government, the dashboard enables real-time monitoring of maternal care, newborn health, immunisation, nutrition, growth tracking, school enrolment, attendance and adolescent health services.
It also generates automated reminders and follow-up alerts while providing real-time dashboards for state, district and taluka-level administrators to support planning and timely interventions.
Officials believe the platform will help identify at-risk children earlier, improve coordination between departments and strengthen the delivery of health, nutrition and education services across Gujarat.
