Poshan Tracker & ABHA Integration Advances Unified Digital Health Records under ABDM
A child’s birth records, immunisation details sourced from U-WIN, and growth monitoring information recorded through the Poshan Tracker application can now be accessed through one ABHA-linked account.
The Poshan Tracker application has been integrated with ABHA under the Ayushman Bharat Digital Mission (ABDM), enabling a unified digital health record system for women and children and strengthening coordination between nutrition and healthcare services.
India has been steadily advancing its digital public health infrastructure through the Ayushman Bharat Digital Mission, which seeks to create interoperable health systems across the country.
ABHA, a unique digital health ID, forms the backbone of this ecosystem by allowing individuals to link and share their health records securely with authorised providers.
The Poshan Tracker application, developed to monitor nutrition services under the Integrated Child Development Services (ICDS) programme, has played a key role in tracking beneficiaries, service delivery, and outcomes at the grassroots level.
With this integration, multiple data streams have been brought together into a single digital profile. A child’s birth records, immunisation details sourced from U-WIN, and growth monitoring information recorded through the Poshan Tracker application can now be accessed through one ABHA-linked account.
This unified approach is expected to reduce fragmentation and improve visibility across different stages of a child’s development.
For pregnant women, the integration has enabled continuity of care by consolidating nutritional support records, antenatal check-ups, and delivery-related information under a single ABHA ID.
This can help frontline health workers and clinicians track progress more effectively and ensure timely interventions when needed.
The move also strengthens real-time monitoring capabilities by enabling accurate and updated data flows across systems. Programme administrators can use this data to identify gaps, track outcomes, and improve service delivery at scale.
The effort is expected to enhance coordination between nutrition and health programmes while supporting improved outcomes for women and children across the country.
