Get Well & RhythmX AI Merge to Launch GW RhythmX Precision Care Platform

The merged platform will serve 150 health systems and 85 million patients, integrating data from EHRs, payers, financial systems, and social determinants to deliver real-time precision care recommendations.

Get Well, a US based patient engagement company, and RhythmX AI, a precision care platform provider, have announced that they will merge to create a new entity called GW RhythmX focused on AI-enabled, patient-centric care delivery.

Both organizations are part of the SymphonyAI portfolio and said the combined company will use artificial intelligence to connect patient engagement with precision medicine and health system performance.

According to the announcement, GW RhythmX will serve 150 health systems and a total population of around 85 million patients, including a significant veteran cohort.

The new platform is described as an AI-driven precision care system that brings together data from more than 10 sources, including electronic health records, clinical guidelines, payer and financial data, and social determinants of health. The aim is to produce real-time clinical and operational recommendations that support clinicians, care teams, and administrators.

Leadership for the merged company has also been confirmed. RhythmX AI founder Deepthi Bathina will lead GW RhythmX as chief executive officer, while Get Well founder and chief executive Michael O’Neil will take on the role of vice chair.

SymphonyAI chair Romesh Wadhwani said the merger is intended to give health systems the scale, data, and technology needed to operate in a more patient-centric, AI-supported precision care environment.

How GW RhythmX Integrates

RhythmX AI’s precision care technology, launched in 2023, has been positioned as an EHR vendor-neutral and is reported to have delivered large volumes of personalized treatment recommendations.

The platform has also been used to predict disease progression, identify possible undiagnosed conditions, and optimize coding and billing at the point of care, linking clinical and financial performance.

Get Well’s digital engagement tools are currently deployed across multiple care settings to support communication, education, and follow-up with patients. The company has previously said its platform has helped reduce hospital readmissions and increase revenue while improving patient satisfaction for health system clients.

Earlier in the year, Get Well introduced an AI-based patient assistant for post-discharge support that was built on RhythmX technology, using automated check-ins and real-time discharge tracking to improve adherence and address common barriers such as transport delays.

By combining these capabilities under a single brand, GW RhythmX is expected to provide health systems with an integrated stack that spans precision analytics, clinical decision support, patient engagement, and workflow optimization.

The focus is on enabling care teams to use unified data for more personalized interventions while supporting health system objectives in quality, efficiency, and financial sustainability across the care continuum.
