Zynix AI, a Florida-based healthtech company developing an AI-first platform for value-based care, has deployed its Care Execution Agents across leading accountable care organizations and health systems, reaching nearly one million patients and strengthening its role in enabling real-time care delivery within value-based care models.
The rollout spans organizations, including Palm Beach ACO, Eternal Health, NHS-affiliated programs, Innovacare, and Privia Health, reflecting growing demand for execution-focused digital health tools.
Zynix AI integrates data from electronic medical records, claims systems, and clinical workflows, enabling AI-driven agents to carry out tasks that typically require manual intervention. These include post-discharge follow-ups, scheduling annual wellness visits, monitoring chronic conditions such as hypertension and diabetes, and identifying and closing care gaps.
By embedding these capabilities into routine workflows, the platform aims to reduce administrative burden while improving patient adherence and care coordination.
