Sutter Health, Epic Launch “Sutter Sync” to Enhance Remote Chronic Care

The program deploys medical devices that connect directly to Epic’s electronic health record (EHR), allowing seamless monitoring without the need for device-specific apps.

Sutter Health has partnered with Epic to launch Sutter Sync, a digital care initiative designed to optimize management of chronic health conditions from patients’ homes.

The program deploys medical devices that connect directly to Epic’s electronic health record (EHR), allowing seamless monitoring without the need for device-specific apps.

Sutter Sync currently utilizes proprietary devices, including a blood pressure cuff, scale, and glucometer, which transmit patient data to the Epic MyChart app on phones or tablets. The information then updates directly into the patient’s health record in Epic, even if the MyChart app is closed or the phone is in another room. This real-time data flow enables clinicians to track patient measurements at home and monitor condition changes between office visits.

The integration provides care teams with actionable insights to make informed treatment decisions and suggest timely medication adjustments remotely. The program is particularly aimed at patients with chronic conditions such as Type 2 diabetes, high blood pressure, and pregnant individuals.

This connected approach can help people better manage chronic diseases, stay healthier, and avoid unnecessary trips to the hospital — all while improving their overall experience.”

Launched in spring 2025 with an initial focus on high blood pressure, the program currently supports over 3,000 patients. Starting in October, Sutter Sync will expand to include eligible Medicare-enrolled patients managing Type 2 diabetes and high cholesterol. A pilot program for pregnant patients is also set to launch with a small cohort of participants and referring providers.

By integrating multiple chronic condition programs under one platform, Sutter Sync aims to reduce patient burden while providing clinicians with comprehensive data to guide care.
