Cleveland Clinic ACO & Remo Health Launch Dementia Support Pilot for Medicare Patients
The collaboration aims to provide comprehensive, home-based care and resources for families across Ohio, especially those in rural areas.
Cleveland Clinic Medicare Accountable Care Organization (ACO) has partnered with Remo Health, a virtual dementia care provider, to pilot a new program supporting Medicare patients with dementia and their caregivers.
The collaboration aims to provide comprehensive, home-based care and resources for families across Ohio, especially those in rural areas.
The program is designed to support patients living with dementia and the more than 450,000 unpaid caregivers in the state, many of whom face challenges such as isolation, health issues, and lack of access to specialized services.
“As dementia becomes increasingly prevalent, it will place significant pressure on our health care system to meet the complex and continuous needs of patients and their caregivers,” said Jessica Hohman, M.D., president of Cleveland Clinic ACO. “We’re excited to embark on this collaboration to deliver wraparound care and support that extends beyond the clinical setting and into the home. We must continue to search for innovative solutions to help guide patients and families through every stage of the 24/7 dementia journey.”
Patients enrolled in the Cleveland Clinic ACO will gain access to Remo Health’s virtual support system and a peer support network of over 20,000 families nationwide, connecting caregivers navigating similar challenges.

This pilot initiative reflects a shared effort to improve outcomes for dementia patients while supporting caregivers who often carry the burden of care with little external help.
