SeamlessMD has announced a new integration with Xealth, enhancing digital health program delivery for hospital systems using Xealth to manage patient engagement.

The partnership aims to eliminate manual workflows and scale digital care journeys more effectively.

With this integration, health systems can now automatically enroll patients into SeamlessMD based on surgical procedures, discharges, or appointments. Teams can also view patient enrollment status directly within the EHR, receive real-time care updates via Xealth, and track patient-reported outcomes and alerts without switching systems.

“By eliminating manual steps, we’re making it easier for care teams to engage patients and improve outcomes, without adding to their workload,” said Dr. Joshua Liu, CEO, SeamlessMD.

The integration supports hospitals already using Xealth to further streamline patient management across surgeries and chronic care, ensuring continuity and improved care delivery from within their existing digital infrastructure.

Days back, we reported that Samsung Electronics was in talks to acquire Seattle-based digital health startup Xealth in a move to sharpen its digital health strategy and integrate patient-generated data with hospital systems.

Founded in 2017 as a spinout of the Providence health network, Xealth enables hospitals to prescribe and manage digital health tools, currently serving over 500 hospitals in the US.

The deal is expected to close later this year, subject to regulatory approvals. Samsung, an existing investor in Xealth, said the acquisition will accelerate its goal of creating a unified platform combining health data from wearables with clinical records to enable more personalized, preventive care. The financial terms are undisclosed.
