Lightbeam Health Solutions, has recently unveiled its AI solution for patient care at the HIMSS25, which occurred from March 3-6, 2025, in Las Vegas, NV.

According to the company, the AI tech offers value-based care (VBC) organizations with essential tools for risk stratification, care coordination, and improved patient outcomes. Key capabilities included predictive analytics, which enabled risk stratification and predicted reduced avoidable admissions while promoting health equity, achieving a 23.6% reduction in readmission risk.

Additionally, Lightbeam presented prescriptive AI insights that facilitated personalized care coordination at scale, analyzing over 4,500 clinical and social determinants of health (SDOH) factors per patient. This analysis yielded deeper insights, enhancing and hyper-personalizing care coordination effectively.

The Best in KLAS deviceless Remote Patient Monitoring (RPM) technology also featured prominently, showcasing scalable AI-powered call and text-based monitoring that expanded care management capacity tenfold and increased patient engagement and compliance.

By that time, Lightbeam had assisted healthcare organizations in generating over $5 billion in value-based care savings.
