Johns Hopkins Launches Illustra to Support Population Health, VBC Goals
Illustra combines analytics, operations, and clinical implementation to convert extensive healthcare data into actionable insights.
Johns Hopkins Health System has introduced Illustra Health, a population health analytics platform designed to support healthcare organizations in managing population health and value-based care (VBC) initiatives.
The platform is currently available to healthcare providers across the U.S.
Illustra combines analytics, operations, and clinical implementation to convert extensive healthcare data into actionable insights. The toolset includes a structured workflow for population health programs, predictive risk scores, and real-time tracking to support decision-making and measure clinical and financial outcomes.
“It aligns clinical, financial, and operational leaders around a shared, data-driven plan – breaking down silos and accelerating momentum,” said Sarah Kachur, President of Illustra. “Rather than waiting for data integrations or struggling to interpret ad hoc reports, teams know where to focus, how to deploy resources, and what results to expect.”
Command Center and Risk Stratification Enhancements
The Command Center offers live insights into top population health priorities and supports resource allocation and risk management. It also delivers tailored worklists for clinical teams and includes dedicated support from public health professionals.
“Clinical teams receive tailored worklists that fit their staffing and workflows, and we provide dedicated support and training from MPH graduates,” Kachur said. “Illustra gives every stakeholder – from care manager to CFO – a connection between strategy, initiative, and outcome.”
A Care Management Targeting feature replaces generalized patient lists with specific, clinically validated groupings that help prioritize interventions. Each entry includes documented reasons and care opportunities, assisting close gaps to missed by traditional stratification tools.
Equity and Impact Tracking Features Included
The platform includes a Health Equity Impact Analysis tool to identify disparities across patient groups and guide strategies to reduce them. It also tracks outcomes over time. Additionally, the Population Health Impact Analysis function enables organizations to measure the effectiveness of interventions across populations and refine strategies accordingly.
