Cigna Outlines Lead to One Strategy, Targets $3 Bn in Savings by 2030

The company expects a parallel operational efficiency initiative, Accelerate to One, to generate $3 billion in run-rate savings by 2030.

The Cigna Group has outlined its Lead to One healthcare transformation strategy, which aims to integrate its medical and pharmacy benefits, behavioral health, and specialty care services to deliver more personalized care and reduce healthcare costs. The company expects a parallel operational efficiency initiative, Accelerate to One, to generate $3 billion in run-rate savings by 2030.

CEO Brian Evanko discussed the strategy during the company's third-quarter 2026 earnings call on November 5. First introduced at Cigna's investor day on September 30, Lead to One is designed to bring the company's healthcare capabilities together through data analytics and technology to identify patient risks earlier and provide targeted support.

Evanko said the strategy will focus particularly on patients with complex clinical needs, who account for more than 50% of total healthcare spending.

The company expects the integrated approach to improve health outcomes while managing costs through more coordinated interventions and personalized support.

Alongside Lead to One, Cigna is implementing Accelerate to One to improve workforce productivity, operational efficiency, accountability and decision-making. The initiative is expected to generate $3 billion in annualized run-rate savings by 2030, with opportunities spread across the organization.

The strategic update came as Cigna reported third-quarter revenue of $73.7 billion, compared with $69.7 billion in the same quarter last year. Net income stood at $1.4 billion, down from $1.9 billion in Q3 2025.

For the first nine months of 2026, Cigna reported $4.8 billion in profit, compared with $4.7 billion during the corresponding period in 2025. Revenue for the nine-month period reached $213.8 billion, up from $202.4 billion a year earlier.

Cigna Healthcare, its insurance business, reported 18.45 million medical members at the end of the third quarter, compared with 18.1 million at the end of 2025. Its pharmacy benefit membership stood at 117.7 million, down from 123.6 million at the end of 2025, reflecting expected client transitions and lower membership among health plan clients.

Meanwhile, Evernorth reported $63.8 billion in adjusted revenue for the quarter, compared with $61.5 billion a year earlier. Cigna Healthcare's adjusted revenue increased to $11.7 billion from $10.7 billion.

Following the quarterly results, Cigna raised its 2026 earnings outlook, projecting adjusted earnings per share of at least $30.60 and a full-year medical loss ratio of 84.2%.
