The new system is powered by Cloudphysician’s RADAR platform, which integrates live data from patient monitors, bedside observations, and clinical workflows into one intelligent interface.

Antara Senior Care, in collaboration with Cloudphysician, has introduced an AI-enabled patient management system at its Bannerghatta Transition Care facility in Bengaluru.

The system offers round-the-clock monitoring for seniors undergoing post-hospitalization recovery and rehabilitation, aiming to improve outcomes and reduce health risks. This marks the first phase of a larger rollout across Antara’s facilities in Chennai, Gurugram, and Noida.

The new system is powered by Cloudphysician’s RADAR platform, which integrates live data from patient monitors, bedside observations, and clinical workflows into one intelligent interface.

By converting complex data into actionable insights, RADAR enables early detection of complications, evidence-based recovery management, and continuous oversight for patients.

The platform ensures seamless coordination between Antara’s on-ground teams and Cloudphysician’s virtual specialists, delivering expert intervention at any time of the day. The use of AI reduces variability and human error, helping maintain consistent standards of care for patients in critical recovery phases.

“Structured post-acute transition care can cut 30-day hospital readmissions by up to ~35%. When powered by AI-enabled care management, transition care can further boost patient safety, reduce recovery costs, and improve efficiency, especially in preventing falls, cardiac events, stroke complications, dehydration, and dementia-related risks.
