Aledade Adds 700 Providers to Value-Based Care Network as Federal Policy Momentum Builds

Aledade partners with independent practices, clinics, community health centers, and health systems across 46 states and the District of Columbia.

Aledade has added more than 700 primary care practices to its value-based care network for 2026, as federal policy developments continue to support the expansion of accountable care models across Medicare and other government programs.

The value-based care enablement company now works with more than 3,000 primary care organizations nationwide, supporting care delivery for over 3 million patients across programs including the Medicare Shared Savings Program (MSSP), Medicare Advantage, Medicaid, and commercial contracts.

The company’s latest expansion positions Aledade as a major participant in the MSSP. In 2026, Aledade is serving one in five new MSSP participants and nearly 20% of the overall program. According to company executives, this growth reflects rising provider interest in shifting away from fee-for-service reimbursement toward value-based arrangements.

Mat Kendall, co-founder and president of Aledade, said providers across geographies and practice sizes are increasingly seeking payment models that reward quality and outcomes rather than volume. He noted growing fatigue among physicians with traditional fee-for-service structures and increased interest in models that support preventive care and population health management.

Aledade reported reaching $1 billion in revenue in 2025 and said it is focused on improving profitability in 2026. CEO and co-founder Farzad Mostashari said the company was profitable in 2025 and expects continued growth in both revenue and earnings.

The company’s expansion coincides with broader growth in accountable care participation nationwide. The Centers for Medicare & Medicaid Services (CMS) reported that 14.3 million beneficiaries were enrolled in accountable care organizations as of January 2026, up from 13.7 million the prior year. CMS approved 134 MSSP applications for 2026, bringing the total number of participating ACOs to 511.

Federal policy shifts are also shaping the value-based care landscape. CMS recently launched the Long-Term Enhanced ACO Design (LEAD) model, which began on January 1 and is set to run through December 2026, following the planned conclusion of the ACO REACH model.

Aledade executives view these developments as favorable for continued adoption of value-based care. Mostashari pointed to recent Medicare Advantage rate proposals and fraud prevention efforts as additional signals of increased federal emphasis on traditional Medicare and accountable care arrangements.

Founded in 2014, Aledade provides technology, analytics, and operational support to help primary care practices participate in value-based programs, with tools designed to integrate into existing electronic health record workflows.
