Innovaccer Launches AI-Driven Readmissions Solution to Cut Costs, Improve Outcomes

The offering integrates Innovaccer’s Data Activation Platform with predictive analytics, agentic AI tools, and mature care management workflows tailored for transitional care management (TCM).

Innovaccer Inc. has launched its AI-powered Readmissions Management Solution. It aims to help hospitals and health systems reduce avoidable readmissions and manage rising costs of transitional care, particularly for Medicare, Medicaid, and uninsured populations.

The offering integrates Innovaccer’s Data Activation Platform with predictive analytics, agentic AI tools, and mature care management workflows tailored for transitional care management (TCM).

The solution is positioned to help healthcare organizations mitigate Hospital Readmissions Reduction Program (HRRP) penalties and optimize hospital capacity.

Over 200 hospitals will face HRRP penalties exceeding 1% of their Medicare revenue in 2025. With national uncompensated care averaging 2% to 6% of operating expenses, health systems are under pressure to contain readmissions that occupy critical inpatient capacity and delay high-margin procedures.

Innovaccer’s solution includes benchmark intelligence using VRDC data, custom dashboards, and agentic AI features like the Care Management Copilot and Pre-Call Coordinator Agent.

Additionally, it is designed to improve documentation and billing workflows while reducing the care manager workload by targeting low-touch patients.
