HealthSnap Raises $25 Mn to Expand AI-Driven Virtual Care Platform Across U.S. Health Systems

US-based AI health startup, HealthSnap, has secured $25 Mn in growth financing to accelerate the nationwide expansion of its AI-powered virtual care platform, which currently supports more than 80,000 patient programs across 200 health systems.
The investment will strengthen the startup's financial position, support the rollout of its Advanced Primary Care Management (APCM) platform, and accelerate the adoption of AI-enabled remote care among health systems.
HealthSnap said the financing, led by Eastward Capital Partners with Ziegler serving as its exclusive financial advisor, will refinance existing debt, improve balance sheet liquidity, and support further commercial expansion.

The startup's platform combines Remote Patient Monitoring, Chronic Care Management, Principal Care Management, and Advanced Primary Care Management within a single electronic medical record (EMR)-integrated system.
The platform currently supports more than 80,000 active patient programs across over 200 health systems and physician organizations, including Prisma Health, AdventHealth, Ascension Health, Sentara Health, Tampa General Hospital, and UnityPoint Health.
HealthSnap expects that number to exceed 100,000 active programs by the end of 2026.
According to HealthSnap, its AI engine processes approximately two patient measurements every second and uses machine learning to filter non-actionable alerts.

HealthSnap reports that patients enrolled in its programs experience a 97% reduction in alert frequency by their twelfth month, helping care teams focus on clinically significant events while reducing alert fatigue.
Beyond patient monitoring, HealthSnap has also expanded the use of AI across clinical workflows. During 2025, its AI system reviewed more than three million clinical progress notes for quality assurance, while AI-based simulations were used to train newly onboarded care navigators and nurses before they interacted with patients.
HealthSnap also uses AI tools to automate routine administrative tasks such as patient enrollment, device troubleshooting, and compliance outreach.
Published studies cited by HealthSnap have reported improvements among high-acuity Medicare beneficiaries, including reductions in systolic blood pressure, a median annual cost-of-care reduction of $10,932 per patient, 40.9% fewer hospitalizations, and a 19.9% decline in emergency department visits.
