Clarify Health Acquires Loyal Health to Unify Referral Intelligence & Patient Activation

Clarify Health has completed the acquisition of Loyal Health Holdings, Inc., a healthcare-specific patient activation platform, creating what the company describes as the industry's first closed-loop network intelligence engine. The combined platform is designed to identify where patients need care, activate them into the appropriate provider, and measure outcomes across the referral journey.

Loyal's Care Activation Platform manages more than 80,000 provider and location profiles and supports millions of patient searches and real-time scheduling transactions annually across health system customers nationwide. Combined with Clarify's Meridian® intelligence platform, the offering provides health systems with visibility into patient access, referral performance, and network growth. Existing customers of both companies will continue to receive the same products, services, and support, according to the companies.

"Health systems know that better referral management drives better outcomes," said Todd Gottula, CEO and Co-Founder of Clarify. "What they haven't had is a single platform that connects the intelligence, where should this patient go, with the activation that gets them there. Clarify provides the intelligence. Loyal provides the activation. Together, we're giving health systems the ability to run the full loop from referral to measured outcome."

The acquisition targets a challenge often referred to as the "referral gap." According to industry research cited by Clarify, an estimated 30% to 50% of referrals never result in a completed visit, costing health systems between 10% and 30% of potential revenue. The company noted that referral management, patient access, and quality initiatives are often addressed through separate systems, creating operational inefficiencies.

The combined platform will integrate referral intelligence, patient activation, and outcomes measurement. Clarify said Meridian identifies referral patterns, leakage, and growth opportunities, while Loyal's platform supports patient outreach, digital engagement, and real-time scheduling. The platform also combines Clarify's machine learning models for quality, cost, and network performance with Loyal's AI-powered scheduling, chat, and predictive propensity tools.

"Loyal has built a platform that close to 500 hospitals rely on to connect patients with the care they need," said Nanette Oddo, former CEO of Loyal Health. "This combination ensures that work continues with the resources, data, and scale to make it even more impactful for our customers and the communities they serve."

Todd Gottula will continue to lead the combined company, while Loyal will operate as a wholly owned subsidiary of Clarify Health. Terms of the transaction were not disclosed.
