Teladoc Health Unveils Outcome-Based Virtual Care Model for Employers and Health Plans

Under Teladoc One, care teams deliver and coordinate care across virtual settings while working alongside members’ local healthcare providers.

Teladoc Health has launched Teladoc One, a new virtual care model for employers and health plans that combines coordinated, personalized care with a payment structure tied entirely to clinical and financial outcomes.

The model is designed to replace fragmented, condition-specific healthcare programs with an integrated approach that supports a member’s overall health needs. Under Teladoc One, care teams deliver and coordinate care across virtual settings while working alongside members’ local healthcare providers.

Initially developed in partnership with a select group of employers and health plans, the model will first focus on populations with cardiometabolic conditions. Programs will launch with an initial group of enterprise customers in September 2026, with broader availability planned for January 2027.

The platform is powered by Teladoc Health’s Pulse intelligence engine, which aggregates clinical history with claims, pharmacy, device, medical record, engagement, and eligibility data. According to the company, these insights enable care teams to identify emerging health risks, coordinate care, support specialty referrals, and improve member outcomes.

Kelly Bliss, President of U.S. Group Health at Teladoc Health, said, “Teladoc One, fundamentally, is a new care model that’s changing the way that we deliver care.”

The company said Teladoc One is built on a primary care foundation that enables personalized care across multiple health needs rather than focusing on predefined condition-specific programs.

According to Ethan Berke, MD, Chief Medical Officer at Teladoc Health, the model is designed to improve condition control, medication adherence, care plan engagement, and follow-up while reducing medical costs, unnecessary referrals, emergency room visits, and hospitalizations.

Teladoc Health said 100% of the model’s fees are at risk, with success tied to validated medical cost savings and improved clinical outcomes.

Patients are supported by multidisciplinary virtual care teams that include licensed clinicians, certified health coaches, registered dietitians, mental health therapists, specialists, and care guides. The company has also incorporated AI capabilities to provide reminders, scheduling assistance, personalized check-ins, and support between clinical interactions.

According to Teladoc Health, licensed clinicians remain responsible for diagnoses, treatment decisions, and direct patient care, while AI supports care teams by helping identify risks earlier, personalize outreach, monitor health changes, and assist with care plan follow-ups.
