Assort Health Launches AI Agent to Automate Specialty Care Referrals
It extracts and structures referral information, applies an organization’s eligibility and specialty-specific criteria, identifies missing information, and matches patients with appropriate providers.
Assort Health has launched Referrals, an AI agent designed to automate the referral process for specialty practices and health systems, from intake and eligibility verification to provider matching and appointment scheduling.
The agent processes inbound referrals from electronic health records (EHRs), faxes, PDFs, and phone calls. It extracts and structures referral information, applies an organization’s eligibility and specialty-specific criteria, identifies missing information, and matches patients with appropriate providers.
According to Assort Health Founder and Co-CEO Jon Wang, the agent identifies a better-fit provider than the one initially selected in 50% of referrals. The company also reports a 79% referral-to-appointment conversion rate, with 87% of referrals fully automated end-to-end.
Referrals can proactively contact patients through voice, text, or email and schedule appointments directly within the EHR. The system also supports ongoing patient conversations during the referral process.
The agent is powered by Synapse, Assort Health’s proprietary model, which the company says was developed using more than 225 million specialty-specific patient interactions, along with 70,000 care protocols and decision pathways.
One healthcare organization using the platform is Chattanooga, Tennessee-based Center for Sports Medicine & Orthopaedics. Call Center Manager Jensine Joseph said the practice previously faced high referral volumes, with initial patient outreach sometimes taking three to four business days and follow-ups extending beyond a week.
The practice now reports that Referrals processes incoming referrals, verifies information, and contacts patients within hours. According to Joseph, about 90% of the practice’s referrals are now automated end-to-end, with a 77% conversion rate.
The launch follows Assort Health’s expansion of its AI product portfolio in 2026. In May, the company launched Activate, an AI agent for proactive outbound patient calling. Wang said the company currently works with several hundred customers.
In June, Assort Health announced a $120 million Series C funding round, bringing its valuation to $1.2 billion.
