Commure & Compassus Partner to Automate Home Care Referrals With AI
The companies reported that the platform reduced referral processing time from about an hour to less than 10 minutes and increased admissions by 6%.
Healthcare technology company Commure and home-based care provider Compassus have partnered to automate patient referrals through Commure Orchestrator, an AI-powered platform designed to match patients with available clinicians based on location and specialty. The companies reported that the platform reduced referral processing time from about an hour to less than 10 minutes and increased admissions by 6%.
Compassus began using Orchestrator in early 2026 following six months of joint development. According to the companies, missed referrals declined by 22% in states where the platform has been operational for the longest period.
The platform consolidates referrals received through fax, payer portals, and email into a centralized system. It then matches patients with available clinicians based on geographic location and clinical discipline, helping intake teams manage referrals more efficiently.
Automating Referral Management
Before implementing Orchestrator, Compassus intake teams navigated as many as 15 separate systems to process referrals, with some referral documents spanning 60 to 80 pages, according to Commure.
The platform integrates with Compassus' existing systems, automatically transferring referral and insurance information to reduce duplicate data entry. It also provides a shared view of each referral's status, enabling teams to track cases throughout the intake process.
Compassus introduced the platform in phases and used feedback from early deployments to refine its workflows.
"Our focus is getting patients home from the hospital and settled into care as soon as they're ready," Evan Kramer, senior vice president of innovation and operational excellence at Compassus, said in a statement.
Commure announced the general availability of Orchestrator in June 2026. The company said the platform can also connect with its call center and digital intake agents to support appointment scheduling and the collection of consent forms, previsit information, and insurance details.
Growing Investment in Healthcare Automation
The partnership comes amid continued investment in AI tools designed to streamline healthcare administration and post-acute care operations.
In May 2026, Commure raised $70 million in a funding round led by General Catalyst, bringing its total funding to $750 million and its post-money valuation to $7 billion. The company said it would use the funding to expand its revenue cycle and practice management platform, enhance its shared intelligence tools, and enter global healthcare markets.
Other companies are also developing technology to improve referral management and admissions in post-acute and home-based care.
In September 2026, Basalt Health raised $20 million for a platform that helps post-acute facilities evaluate hospital referrals and determine admission eligibility. Its software also supports benefits verification and prior authorization.
In May 2026, Enzo Health secured $20 million to expand its AI-powered home health platform, which reviews documentation for reimbursement and compliance and provides referral recommendations based on coverage and eligibility. The company also outlined plans to expand into skilled nursing and hospice care.
